Moreland School District is excited to once again partner with Stanford Children’s Health Teen Van to offer Covid-19 booster shots for students 12 years of age and older. There is no cost and no insurance required.

If you are interested in having your child receive a free Covid-19 booster shot, please sign-up here. (Please note: this is not the 2nd series of the Covid-19 vaccine. This is a booster shot for students who have received the full series of shots at least 5 months ago. If you are not sure of your child’s eligibility, you can learn more here.)
Parents must bring the TWO signed consent forms included with this letter.
Parents/students should bring the child’s CDC COVID-19 Vaccination Record card to the booster shot appointment so the provider can fill in the information about your booster dose. If you did not receive a card at your first appointment, contact the vaccination site where you got your first shot or your state health department to find out how you can get a card.
Parents will report to their school’s office to check-out their child 15 minutes before the time of their appointment. After the booster shot is received and the student feels healthy after the 15 minute waiting period, students may return to class. Please have your child return to school through the front office.
All health information collected at the time of your child’s visit will be kept confidential and will not be shared with the Moreland School District. Your child will receive their Covid-19 booster shot directly from Stanford.
